Continuous Glucose Monitors Help Type 2 Diabetes Patients, Trial Finds

For adults living with type 2 diabetes who do not use insulin, managing blood sugar has traditionally meant finger-prick tests, guesswork between appointments and a three-monthly lab result that arrives long after the meals that shaped it. A major clinical trial now suggests a wearable sensor can change that picture in a measurable way — helping patients spend about five more hours a day in their target blood sugar range.

    What the Trial Found

The study, described as the largest clinical trial of its kind and published in the New England Journal of Medicine, followed adults with type 2 diabetes who were not on insulin — a group that has historically had limited access to continuous glucose monitors, or CGMs. Participants using the devices spent approximately five additional hours each day within their target glucose range compared with usual care.

Five hours is not a marginal gain. Time in range is increasingly treated by clinicians as one of the most meaningful day-to-day measures in diabetes care, because it captures the highs and lows that a single average figure can hide. Reporting from the University of Colorado Anschutz Medical Campus outlines how the trial was designed and why researchers focused on patients who do not use insulin.

    How Continuous Glucose Monitors Work

A CGM is a small sensor, usually worn on the arm or abdomen, that measures glucose in the fluid just beneath the skin every few minutes and sends the readings to a phone or handheld reader. Instead of a handful of snapshots a day, patients and clinicians get a continuous line — complete with trend arrows showing whether glucose is rising, falling or holding steady.

That feedback loop is where the benefit appears to come from. A wearer can see, within an hour or two, what a particular meal, walk or stressful morning did to their glucose, and adjust the next decision accordingly. Over weeks, those small corrections add up. It is the same principle readers will recognise from our article on subscription fatigue and monthly budgets: what you can see clearly, you can manage — and what stays invisible tends to drift.

    Who Stands to Benefit Most?

The clearest beneficiaries are the patients in the trial’s own profile: adults with type 2 diabetes managing their condition with lifestyle measures or oral medication, who previously had no routine way to see their glucose patterns. For them, a sensor can reveal problems that finger-prick testing misses, such as overnight lows or sharp post-meal spikes.

Clinicians may benefit too. A fortnight of sensor data gives a far richer basis for adjusting medication than a single lab value, and it can make consultations more collaborative — patient and clinician looking at the same graph, discussing the same meals and routines. Guidance from organisations such as the National Institute of Diabetes and Digestive and Kidney Diseases explains how CGM data is used in everyday diabetes management.

    Barriers That Still Need Solving

None of this makes sensors a universal answer. Cost and insurance coverage remain the biggest obstacles, particularly for patients not using insulin, who in many health systems have been last in line for reimbursement. Sensors also generate data that someone has to review, and clinics need time and systems to do that well.

There is a human factor as well. Some wearers find constant readings reassuring; others find them stressful. Good programmes therefore pair the device with education and support, rather than handing over a sensor and hoping for the best. Our guide to improving the mobile gaming experience makes a similar point about wearable and mobile technology in general: the hardware only delivers when the experience around it is designed for real people.

    What Happens Next

Evidence of this scale tends to influence guidelines and coverage decisions, though both move slowly. The trial strengthens the argument that CGMs should not be reserved only for people using insulin, and it gives payers a concrete outcome — more hours a day in a healthy range — to weigh against the cost of the devices.

For patients, the practical advice is simple: if you have type 2 diabetes and have never discussed a glucose sensor with your clinician, this is a good moment to ask. The technology is proven, increasingly available, and — as five extra hours a day in range demonstrates — capable of making a daily difference to a condition that is managed, above all, day by day.
